Adding Records

Uploading a Multi-Page Document

Photograph a paper document page by page and keep it together as one organized set in your Vault.

Discharge summaries, lab panels, and insurance paperwork rarely fit on one page. With image sets, you can photograph a paper document page by page and Mediphant keeps the pages together, in order, as a single document - perfect for multi-page records, receipts, or forms.

Capturing a multi-page document

  1. 1

    Start an upload

In your Vault, tap Add and choose Add a document / image set from the Add Files menu.

  2. 2

    Add your first page

You'll land on the Capture Document screen: “Add photos of each page in sequence.” Tap Add Photos and take a photo of page one with your device's camera, or pick an existing photo from your library.

  3. 3

    Keep adding pages, in order

Each photo appears as a tile labeled Page 1, Page 2, Page 3 and so on. Tap the dashed Add page tile to capture the next page. Pages are saved in the order you add them, so photograph front to back. Made a mistake? Remove a page with the small ✕ on its tile and re-take it. You can add up to 10 pages per set.

  4. 4

    Save the set

When every page is captured, tap Save image set. You'll be asked to name your document - something like “Lab Results” or “Hospital Discharge – March.” Naming is optional, but a good name makes the record much easier to find later.

  5. 5

    Review and upload

Your set drops into the Review Files screen as one grouped document. Tap Upload, and Elly reads every page together as a single record - so a value on page 3 is understood in the context of page 1.

For the cleanest results, lay the page flat in good light, fill the frame, and avoid shadows from your hand. Elly can read most photos, but a clear photo means a more accurate record.

Already took the photos? Group them afterwards

If you select several photos at once through Add photos, Mediphant shows the Organize Photos step: “Group related images into a set, or continue and upload them individually.” Tap the thumbnails that belong together - a checkmark appears on each - then tap Create set from selected. You can rename the set with the pencil icon or ungroup it with the ✕ before uploading.

Common questions

Up to 10 pages per set. For a longer document, split it into two sets - for example “Surgery Packet 1 of 2” and “Surgery Packet 2 of 2.”
Not yet - pages keep the order you added them in. If a page is out of order, remove it with the ✕ and add it again in the right position.
No problem - it's saved with the date and time it was captured, and you can rename it any time from the Review screen or your Vault.
Only in where it starts. If you already have a multi-page PDF, just upload it directly - image sets are for paper documents you're photographing yourself.
